Covenant's unique approach to quality assurance and
performance improvement is evidence based
and we want to share that framework with you!
Our QAPI framework, which is implemented successfully at the St. Pete Pier, BayCare Health System, and multiple commercial office properties, ensures that Covenant officers will operate under consistently monitored and continuously improving standards. Supervisors and managers enforce policy, lead training, provide coaching, and ensure adherence to all post responsibilities. Let’s get started with the basics of QAPI.
What is QAPI?
Quality Assurance and Performance Improvement (QAPI) is a framework developed by the Centers for Medicare & Medicaid Services (CMS) to help healthcare organizations, especially nursing homes and hospitals, systematically improve care quality and safety. It merges two complementary disciplines: Quality Assurance (QA) and Performance Improvement (PI).
Core Concept
QAPI is a data-driven, organization-wide approach that ensures continuous improvement in care delivery and resident outcomes. It’s not just compliance, it’s a culture of accountability and learning. Together, QA and PI form a cycle of measurement, analysis, and action that drives sustainable quality.
Quality Assurance (QA)
ssFocuses on maintaining standards and ensuring care meets regulatory and professional benchmarks. It’s both retrospective (reviewing past performance) and anticipatory (preventing future issues).
Performance Improvement (PI)
Focuses on continuous process improvement, identifying root causes of problems, testing new solutions, and measuring results. PI aims to make good care even better.
CMS FRAMEWORK - THE 5 ELEMENTS OF QAPI

Regulatory Foundation
Under 42 CFR §482.21, hospitals and nursing facilities participating in Medicare or Medicaid must maintain an ongoing, data-driven QAPI program.
- Measuring and tracking indicators related to health outcomes and medical errors.
- Using data from internal sources and CMS quality reporting programs.
- Demonstrating measurable improvement in care processes and resident quality of life.
- Engaging all departments and contracted services in the program. Centers for Medicare & Medicaid Services eCFR IntelyCare

Why It Matters
- Reduces adverse events and hospital readmissions
- Improves resident satisfaction and staff engagement
- Strengthens transparency and accountability
- Aligns with CMS’s broader goals for value-based care
Practical Example
A nursing home identifies frequent medication errors.
- QA reviews incident reports and ensures staff follow protocols.
- PI analyzes workflow, retrains staff, and tests new electronic medication tracking.
- Over time, error rates drop and resident safety improves, demonstrating QAPI in action.
